New Applicant Requirements
Applicant Requirements (Original documents required)
- High school graduate (sealed official transcript required) or GED/HiSet
- transcripts or official transcript evaluation of high school equivalency (for foreign transcripts)
- Valid California Driver's License or ID
- Social Security Card - If not a citizen, valid Permanent Resident ID
- No felony convictions
- Completed and Passed “Intro to VN” Class
Medical documents required
- Proof of negative TB test or negative chest x-ray (within the last year)
- Proof of Hepatitis B vaccine
- TDap (vaccine within the last 5 years)
- Proof of Varicella vaccine or titer
- MMR vaccines or titers
- Physical examination signed by MD, NP or PA stating no physical lifting
restrictions; (within 6 months of submission date) - Proof of flu vaccine (season only)
- Copy of American Heart Association CPR card-BLS
(only American Heart Association will be accepted)
